If the clinician deems it appropriate and feels you need to have treatment that cannot be done in the practice they will refer you to a hospital, clinic or another GP surgery that can treat you.
For some specialities (such as skin, ear nose & throat, eyes, etc.) you will be referred to a "GP with Specialist Interest" (GPwSI), this service will usually be at another general practice. The GPwSIs can treat many problems themselves, but if you need referring to hospital for advice or treatment they will offer this instead of reffering you back to your GP.

It is the hospital you have been referred to who arranges your transport (previously this was done by the surgery).
If you require transport to the appointment you've been referred for, you should contact the hospital directly. However, this is only available where there is a medical need, and cannot be arranged for other reasons.
If you know you are going to need transport, you should inform the clinician at the time of appointment as they will need to note this on the referral. If this isn't done the hospital will not be aware that you need transport and the reason you need it.
