Phone Lines

We are currently experiencing a high volume of calls to the practice.  Please remember between 8am -10am each day we are only taking requests for appointments for GP's. Any queries or test results we would ask you call the surgery after 10am to help us so we can deal with those patients requiring an appointment.

Contat Details

It is very important that the practice has your current email address, telephone number and address so that we have the correct information on your record and more importantly so we are be able to contact you in times of emergency or for routine matters.

If your address or telephone number has changed then the practice needs to be informed as soon as possible.  You can either do this via filling out a simple form at the surgery or click here and fill out the change of contact details form online.

Please note if you are using this form to change your name you will be require to support this with legal documentation at one of the sites.  

Sick/Fit Notes

Please can we ask that all sick/fit note requests are put in writing using our Patchs non clinical request and not requested via our phone lines.

Medication Requests

Please continue to order via System online, at the surgery using the paper requests or online via our Patchs non clinical request.  Please see below the details of the PATCHS SYSTEM.

Cervical Screening (Smear Test)

If you're due your smear test, please contact the surgery on 01274 484111 to book in with one of our practice nurses.  If you would prefer an appointment out of of hours due to work, other commitments, etc. please let the practice know as we can offer out of hours appointments at other designated clinics.  Please click here for more information.

HRT Prepayment Scheme

We wanted to make you aware of a new scheme which will reduce the annual cost of HRT medication for those who pay for their prescriptions. It is called the HRT Prescription Prepayment Certificate (HRT PPC). The HRT PPC will cover certain HRT medicines for twice the single prescription charge and will be valid for 12 months. It will save you money if you need to pay for 3 or more HRT prescription charges within 12 months. You can check whether your HRT is covered by the scheme and register by using this link.

Minor/Mild illness or Injury

 
NHS services across the North East and Yorkshire region are experiencing extremely high demand at the moment. This includes GP practices and the 111 service which are both extremely busy.
 
If you need advice for a minor/mild illness or injury, in the first instance, please either contact your local pharmacy for help or look at the self-care advice that’s available on-line from the NHS at www.nhs.uk
 
If you want to check your symptoms, there is an on-line symptom checker available at 111.nhs.uk, which will re-direct you to a health professional if you need to be consulted by someone.
