Innovation

Vitrucare

We are a pilot site for Vitrucare which is a tool to aid in the management of long term conditions such as diabetes, heart disease, high blood pressure and repiratory diseases such as COPD and asthma.

It helps put you in control of your health to manage your own goals whilst monitoring your outcomes and informing your GP should things go off track.

If you wish to be involved please let us know. You will of course be offered the opportunity to take part when you are next reviewed at our practice. We hope this tool will allow us to target our resources more appropriately and avoid unecessary trips for you to the surgery.

Long term conditions management

We are restructuring our long term conditions management and this will be rolling out to Carlton patients ove rthe next year. This work is headed by Prof. Shahid Ali and will provide you with a much more tailored service but we hope will less frequent visits.

Wireless Technology for Patients

This is part of a wider roll out of new technologies by our CCG and as well as providing wireless access for patients at the surgery will provide us with a valuable infrastructure for delivering general or tailored (if you wish) health information directly to you.

Patient on-line

This is a national project & will deliver its first part by the end of March 2015 and the second by the end of March 2016 . It will enable you to access a form of your medical record should you wish. Please enquire at reception regarding gaining access.

Using technology to improve access

We always strive to improve access by effective demand management. Access to appointments in General Practice is always an issue with patients and GPs alike and is a problem nationwide. This is primarily due to the Government's underfunding of the NHS compounded by their recent drive to defund General Practice despite hailing it as leading the way in delivery of healthcare in the 21st century. We are left trying to manage this massive inconsistency. To date we have looked at various triaging and demand management options but remain unconvinced as to their merit. We will continue to explore options in demand management. In the meantime we may be able to offer an alternative consultation option via Skype which has recently become available via our clinical system. This is yet to be implemented across our patch as the solution is awaiting approval but we may be able to offer virtual consultations in the near future which we hope will make accessing your GP at least a little easier.
